How soon can therapy start?
In most cases, therapy begins within 24 hours of approval.
What equipment is needed at home?
We bring everything needed. If any adjustments are required for your safety, our physical or occupational therapists will recommend them.
Can I switch from outpatient to home health rehabilitation?
Absolutely. If travel is difficult or you’ve been discharged from outpatient care, we can help you transition smoothly.
What happens if a patient’s condition changes during care?
We closely monitor each patient’s condition and adjust care plans as needed. If there’s a significant change in the patient’s health, we will notify you immediately and work with you to update the care plan accordingly.

- Sending the referral via fax
- Refer a patient by calling us
- Sending your referral by email
- Immediate Contact – Our team will contact the patient or their family within 3 hours to discuss their needs.
- Start of Care – We perform nursing or therapy evaluations within 48 hours and promptly create a personalized care plan, which will be sent to the Physician for certification.
- Ongoing Communication – We inform Doctors about the patient’s progress, medication changes, critical or any other updates.
