Can I switch from outpatient to home health rehabilitation?
Absolutely. If travel is difficult or you’ve been discharged from outpatient care, we can help you transition smoothly.
What happens if a patient’s condition changes during care?
We closely monitor each patient’s condition and adjust care plans as needed. If there’s a significant change in the patient’s health, we will notify you immediately and work with you to update the care plan accordingly.
How does the referral process work?

- Submit a patient referral
- Using the Morecare or Wellsky referral support service at the hospital or clinic
- Sending the referral via fax
- Refer a patient by calling us
- Sending your referral by email
- Immediate Contact – Our team will contact the patient or their family within 3 hours to discuss their needs.
- Start of Care – We perform nursing or therapy evaluations within 48 hours and promptly create a personalized care plan, which will be sent to the Physician for certification.
- Ongoing Communication – We inform Doctors about the patient’s progress, medication changes, critical or any other updates.
